For help filling out the form, click the link to the left of each blank. Push the button, and copy the text from the lower window to your word processor.

When you're finished creating your individual items, you'll still have to format your "works cited" list properly.

1. Alphabetize all entries by author's last name.
2. Double-space (don't skip extra lines between entries).
3. Use the hanging indent paragraph format.
