My header keeps appearing in every numbered page, how do I fix that.
You may have typed the title block in the space where the header goes. The title block is just at the top of the first page, not above the top margin where the header goes.
How i can complete MLA format about above website?

This page on the Purdue website does a good job presenting the general form. Find an authoritative publication of the text, preferably in the sheet music sold by the publisher -- plenty of online sites get the lyrics wrong. If not, cite a CD or MP3 recording, which would have a publisher and date.
